Forms to Complete Member Requirements

Below are the forms that academy members will need to use in order to complete each of their co-curricular requirements. All documents will be turned in virtually to the Google Classroom.
 
Co-Curricular Requirements

Verification Forms

Each year, students will need to complete the appropriate verification form and get it signed by a supervisor for each activity to confirm completion of that activity. There is a section for each requirement as needed by year.
 

Volunteer Experience and Reflection

Each year, students will need to spend time volunteering at a specific event. We don’t require a particular type of event or specific amount of time, but it is suggested that you dedicate at least 1 hour of volunteer time. The purpose of volunteering is to give back to the community and contribute to making it a better place for everyone. Please ensure that an adult signs your verification form after the event, and then take a moment to reflect on your experience and answer the questions on the Volunteer Experience Reflection Doc.
 

Healthcare Book Reading and Reflection

During an academy member's Sophomore and Junior years, they will need to read a healthcare-related book and write a reflection on how that book has impacted them. The purpose of reading the book is two-fold. First, we want students to learn from others’ experiences in the healthcare field, and this is a great way to do just that. Second, the healthcare field is constantly advancing and incorporating new technologies and medicines; therefore, it is a healthcare worker's responsibility to stay up-to-date with such advancements by reading about them. We hope to encourage this continued education by getting members to read more.
We have a list of pre-approved books, which also indicates whether the book is available in the school library, the Mukwonago Community Library, or through the Sora app. A student may choose a book not on the list, but an AMP Advisor must approve it. Upon completing the book, students will need to write a one-page reflection. The details of what to include in the reflection are in the AMP Book Reflection Prompts doc.
 

Job Shadow and Reflection

During their junior and Senior years at the academy, members will need to conduct a job shadow at a local business of their interest. The reason we want members to conduct job shadows is to gain valuable experience in the fields they are most interested in. These experiences can be eye-opening, helping students understand what these jobs look like on a day-to-day basis. They can also provide excellent connections for students to make with others in their field, which they can draw upon in college and beyond.
For the job shadow, students will set this up themselves. It is their responsibility to find a business that interests them and reach out to schedule the job shadow. Advisors do have some connections if students are struggling to make it work. Students need to have the professional sign the verification form during the job shadow, and then the student will complete a reflection based on the experience. The job shadow reflection guide provides key ideas to focus on, along with questions to facilitate reflection. It may be helpful for the student to read through this before the job shadow to know what to look for and to develop questions to ask.
